All you care to eat--pancakes, French toast, scrambled eggs, hash browns, ham, sausage, coffee, milk, orange juice. Just $6.50 for adults 12 & older and

Sarpy County Cooperative Head Start Round Up

2017-2018 School Year

Early Head Start serves children from birth to age 3 and pregnant women. Families need to must meet the eligibility requirements. EHS has a home-based option, OR a center-based option for families who are working or going to school.

Head Start is a pre-school program that serves families with children, aged 3 by July 31^st and not 5 before July 31^st.

ROUND UP WILL BE CONDUCTED ON THE FOLLOWING DAYS:

Thursday, March 9^th, 2017 ___ Tuesday, March 14^th , 2017

Fort Crook Elementary Parkview Heights Elementary

Bellevue 4:00-6:00 pm La Vista 4 :00–6:00 pm

**The child does not need to attend**

**Transportation resource list available from staff upon enrollment*

Please bring the following information:

Income Verification: 2016 Tax return-1040 (preferred), or one the following: W-2’s, TANF

or SSI, past 12

months of pay stubs, letter from employer on company letterhead (must state the hours

worked,

hourly salary, date of employment, and include the name and phone number of the company).

Submit all income including child support and/or unemployment payments.

Also bring: Child’s Birth Certificate - Address verification for Sarpy County-

Immunization Record

Print out the application at http://www.sarpyheadstart.org and bring the completed application and the above named documents to the express lane at the roundup.
